Dorset local authorities and clinical commissioning group (CCG) are reviewing the dementia service provision in Dorset and how the dementia service can be improved. They want to make sure that dementia services help people maintain independence, choice and control over their lives.
If you or your family have accessed any services related to dementia, there is a feedback survey that will help towards developing the new service model for dementia. The survey closes on 30th November 2016 with the report due out in 2017.

- Community Mental Health Teams
